Birth Record Application

The Registrar's Office issues and maintains birth certificates for unincorporated communities only. Villages have their own clerk and vital record offices. Facilities under Hempstead Town's jurisdiction are:

• LIJ Valley Stream, formerly Franklin General Hospital and Franklin Hospital Medical Center in North Valley Stream
• * Mercy Hospital in Rockville Centre (partial records maintained by Hempstead Town)
• Mitchel Air Force Base in Uniondale
• Mount Sinai South Nassau in Oceanside, formerly South Nassau Communities Hospital
• Nassau University Medical Center in East Meadow, formerly Meadowbrook Hospital and Nassau County Medical Center

*This Office maintains some records for Mercy Hospital including records prior to August 1931 and records from February 24, 1941, to October 2, 1941. All Mercy Hospital records from October 2, 1941, to the present are filed with the Village of Rockville Centre Registrar. Requests

Birth records may be requested by the person named on the record if they are over the age of 18, or by the parents of the child, a legal representative, or a government agency by the following ways:

In person requests

Applicants should bring the following to the Town Clerk's office:

• A completed application
• Valid Government ID
• The appropriate fee

Mail requests

Applicants must send the following:

• A completed application
• Valid Government ID *( Address on ID should match mailing address, if different please include proof of address )
• The appropriate fee

Types of acceptable identification include valid driver's license, valid non-driver's license, valid passport, naturalization papers, military ID, employer's photo ID, two current utility bills showing applicant's name and address, or a police report of stolen ID.

There is a $10 fee per certified copy of a birth certificate. Acceptable forms of payment include cash, money order, local check, or credit card ($1.55 service charge when paying in person). Please make checks or money orders payable to Hempstead Town Clerk.

Pre-Adoption Birth Certificate Notice

Pursuant to New York State Public Health Law 4138-E, effective January 15, 2020, adoptees who are 18 years of age or older may now request a pre-adoption birth certificate.

We want to ensure that you receive your pre-adoption birth certificate as expeditiously as possible. With that goal in mind, we suggest that you apply directly through the NYS Department of Health, as local registrars may not be able to confirm the information required to issue pre-adoption certificates.
